We look forward to seeing you. Please bring your valid ID, insurance information, and credit card to your appointment.

Please note: we require all patients to keep a valid credit card on file (encrypted) for faster checkout and efficient billing.

Payments

We require full payment on the day of service for all patients

For medical appointments, please be aware of your insurance deductible balance prior to your appointment.  Upon checkout, you will pay the total estimated cost of your treatment as defined by your specific insurance plan, plus any remaining deductible, co-insurance, and co-pay. 

We will submit your claim to your insurance company if applicable.  After insurance processing, any overpayment will be immediately refunded/ any remaining balance will immediately be invoiced via email. 

For your convenience we accept cash, all major credit cards and HSA/FSA accounts. Checks are accepted under $200. 

Appointment Cancellations

We understand that things come up and we are happy to cancel or reschedule your appointment with sufficient notice. This allows us time to reallocate your appointment time to other patients on our waitlist.  Please see Financial Policy for more information. 

  • Medical missed appointment/changes with less than 24-hr notice will incur a $75 fee.  

  • Cosmetic/ Surgical missed appointment/changes with less than 48-hr notice will incur a $100 fee. 

New Patients

We are a paper-free office so all new patient paperwork will be completed electronically at your first visit.  If you need any assistance, we are happy to help. 

Please arrive 5-10 minutes ahead of your visit to complete check-in so your appointment can begin on time. 

Please bring with you:

  • Valid Photo ID

  • Insurance Cards (primary & secondary)

  • Valid Credit Card - to be kept on file (see Financial Policy)

  • HSA Card 

  • List of Current Medications

Patient Portal

After your first appointment you will be emailed a link to set up your online Patient Portal account. Open the emailed link to set up your username and password.

This portal will allow you to update your health history, check for biopsy results, and confidentially communicate with our staff. If you are having trouble with your portal account, please call our office. 

Illness

We urge patients who are feeling ill with fever, cough, or other respiratory symptoms to please reschedule their appointment; please provide 24 hours notice so we can appropriately manage our staff and schedule.
